This paper applies a novel casualty analysis methodology to analyse an ice incursion incident that happened with the Floating Production and Storage offloading vessel operating in the White Rose oil field, Canada - Newfoundland. The FPSO named SeaRose did not disconnect and sail away when a medium iceberg came too close of the FPSO. Based on the timeline of the events that happened in the incident, human and organizational factors are coded. Further, important aspects of the incident consisting of causal factors and root causes are identified using the taxonomy. The study provides information on what changes are required in order to prevent incidents related to ice incursion and allision, particularly with respect to operational safety, management and training of individuals.
