An analysis identifying the basic causes of fluid releases in Floating Production Storage and Offloading (FPSO) units is conducted based on 321 spills or releases events in FPSO units occurred from 1992 to 2005 in UK continental shelf. First, the components involved in spills or releases are identified. The frequencies of releases from this system level are presented with the objective of identifying the release prone areas and system elements. An analysis is performed highlighting the causal factors of releases from components in terms of design errors, components failures, operational and procedural errors. Statistics are presented in order to prioritize basic causes and to formulate relationships of releases with FPSO components. Such analysis may indicate the need for improvement of safety standards and system design pertaining to FPSOs.
