The paper presents a new approach for analysis and coding of maritime casualties. It is a result of an EC sponsored research project CASMET. The primary motivation for initiating the project was the lack of focus on human and organisational error and the highly differing investigation practices in EC countries. The paper proposes the requirements for an ideal analytical approach and assesses how these requirements are satisfied by a selection of well known methods. The CASMET approach is not an entirely novel approach and does rather combine the best from different approaches. The method has been tested with fairly good results on rather complex casualties like the capsize of HERALD OF FREE ENTERPRISE.
