The paper extends the conventional failure mode and effects analysis (FMEA) methodology by considering costs of failures into generating risk priority numbers as a basis to finalize the failure analysis of a floating offshore wind turbine at both components and at systems levels. Accordingly, critical systems as well as components of the floating offshore wind turbine are ascertained. Recommendations on corrective designs, e.g. implementation of robust design to the pitch assembly, and preventive actions on safe operations, e.g. monitoring vibration of the gearbox and temperature of the converter, are concluded. The results of this analysis are validated by a comparison with other studies published related to failure analysis of offshore wind turbines.
