The paper presents a consistent procedure for macroscopic context quantification and assessment of cognitive error probability of the individual performance in accidental conditions. Context quantification consists in the macroscopic description of the human-machine system by counting identical contexts. The procedure consists of consecutively applying of the methods of ‘Violation of Objective Kerbs’, and Combinatorial Context Model for context quantification. Their results are used for obtaining the cognitive error probability by the Performance Evaluation of Teamwork method, which is based on Rasmussen’s Step-Ladder Model. The procedure distinguishes three basic concepts determining the reliability of individual human performance: violation, cognition and execution errors. The procedure is illustrated by detailed analysis of the accident "Grounding of Exxon Valdez".
